Team Managers
Please check the website weekly and urge your players to check as
well. On occasion, schedule changes
happen during the middle of a season. A Team
Roster must be completed for your team and turned in before
the first game of each season. All players must have signed a Waiver
of Liability.
Player Cards
Identification cards must be obtained for all players. All players
must have a current player card or sticker in order to play. Cards
must be renewed by Sept. 1 of each year. A picture I.D. is required
with date of birth. There will be a flat rate of $75 for new teams
participating at SoccerPlex for your team's I.D. cards. Replacement
cards are $5. Temporary cards may be purchased for $5.00.
Team Roster
All teams must submit a Team Roster to
Soccerplex management before the first game of each season.
Waiver of Liability
Fees paid to SoccerPlex do not include insurance for injuries to
players, coaches, or managers. All players must sign a SoccerPlex
Waiver of Liability. Parent or legal guardian
must sign a SoccerPlex Waiver for players under age 18.
Youth and Adult Teams
Shinguards are mandatory and must be covered by socks.
Youth Teams
All youth teams must have a responsible adult coach at the time
of league registration.
Adult Teams
Adult league players must be at least 16 years old to play. Proof
of I.D. is required.
Registration
Please complete the Registration Form.
A payment of $100 must be included with your team registration to
secure a league spot. All money is payable to Bally Total Fitness.
Deposit is not refundable if a league is available. The balance
is due prior to the 2nd league game. Visa/Mastercard is accepted.
Referees
All Soccerplex referees are fully licensed professionals managed
by Eric A. Beck.
